Check with your state insurance commissioner–no private company engages in such income/payment structure that I know of, but your state may have some such plan.

Just like you can't buy a new Lamborghini off the lot, based on your income rather than the cost of the car, you can't get health insurance based on your income - EXCEPT for Medicaid - welfare health insurance. If you qualify.

Medical insurance carriers have to file their rates with the state and do not charge based on income. Sorry, rates are what they are; they are based on age & health condition(s).
Previous answer was right on: check with your state Medicaid - best option.
BEWARE of "cheap insurance" plans because you can run into a-la-carte/incomplete, and often not full insurance plans.
