Look on your insurance card. It is usually 10.00, 15.00 up to 25.00 or call your insurance carrier

It depends on whether the doctor/hospital is in the network or not…it also depends on what type of PPO you have, you should probably call your insurance company and they can give you a more exact answer based on who the doctor is. If it is a 70/30 you would pay 30 percent of the allowed amount (the allowed amount is what the insurance company allows the dr to charge). But you may also have a deductible that will need to be met. Call your insurance company to find out for sure!
