i am pregnant, i have no insurance and my husband & i are sep. what does the medicaid office need from me?

i have had 3 miscarriages, and i am supposed to be on progesterone injections. what is the best way to get a medicaid card? what all will the kentucky office need from me? will it need proof of seperation because i don't have one, i can't find my husband, and i don't know how to get one

    Proof that your pregnant from a doctor, I.D., and social security.

    To get a medical card all you will need to do is fill out what is called a Healthy Start Healthy families application, it will ask information such as your income to determine if you are elligible and so forth. Most likely you will be. It will completly pay for everything.

  4. i dont think they will need anything like that. i know they will need proof of address, id, proof of income, maybe your birth certificate and ss card…um…im trying to remember what i needed. i have a medicaid card too. and i'm pregnant right now and have had a really rough pregnancy so i'm there for you. if you ever want to talk email me at crazed_fem19@yahoo.com

    go to your local dept of humal severces and fild for medical secercies as well let them know you are no longer with him that he left and you dont know where he is at it would not hurt to have signed papers from other people who know he isnt no longer in your home with there telephone number with this so they can call them as well take someone with you who will also sign in thee office he no lonnger lives with you ?

    Call your medicaid office before going in, they should tell you everything you need. If he's left you and you can't find him, then usually a sworn affidavit is all you need, they might require a home visit or statement from a former boss. Just call them and they'll tell you what they need. You can also see if a doctor will give you the injections or try a vaginal suppository, that's what I had to take and they cost only like $40 because neither insurance or medicaid paid for them. Medicaid will eventually pay all your medical bills that are related to pregnancy if you qualify.
