When this gets reported to your insurance company, they use ICD-9 codes. The codes are often somewhat vague, but they may paint an unflattering picture. 3 options -
1) Treat it outside the practice
2) Insist on cash payment
3) Have the physician report visit as under different criteria.
You should be able to request to pay cash at the time of visit so the bill never goes to your insurance. Unfortunately, the insurance companies like to know what they are paying for and with all of the fraud that takes place with insurances, I would bet that you would not be able to do it very discreetly.

You could go to an annonymous or confidential clinic and pay for the service yourself. Often these types of clinics offer services on a sliding scale…

yes you can go to the free health clinic, just don’t tell them that you have insurance

You could go to Planned Parenthood. They have sliding scale fees. You could just pay for the service out of pocket.
